Each student has the right to accurate and easy to understand information about their plan of care.
Each student has the option to choose a medical care provider in Student Health Services
or another medical provider of their choice.
Each student has the right to considerate and respectful care from the staff of Student
Health Services.
Each student has the right to know their treatment options and take part in the decisions
about their care.
Each student has the right to talk privately with a health care provider in Student
Health Services and to have their information protected. Each student has a right
to request a copy of their health care record. (A signed release of information is
required).
Each student has a right to a fair, fast and objective review of any complaint they
may have against Student Health Services and its staff.
Each student has the right to access Student Health Services during posted business
hours. In case of an emergency the student is encouraged to contact Emergency Medical
Services at 911 or campus police at 229-430-4711.
Patient Responsibilities
Each student is responsible for submitting accurate and up to date immunization records.

Each student is responsible for providing accurate information about current illness.
Each student is responsible for maintaining compliance in plan of care.
Each student is responsible for following up with provider during plan of care.
Each student is responsible for any medical expenses incurred off campus.
