Data Request Form

In order to balance the multiple requests we receive, the Office of Institutional Research and Effectiveness (OIRE) has developed a Data Request Form. This form will help us identify the specific needs and requirements for requests and will also assist us in organizing these requests for more effective processing. Requests will generally be answered within one week depending on complexity. In general, data is provided in the aggregate and in compliance with guidelines set forth by the Family Educational Rights and Privacy Act (FERPA). Please give us as much notice as possible when requesting data for some requests take longer than others depending on the nature of the request. Please complete all sections of the Data Request Form. Upon completion, submit your request to the OIRE by clicking on the Submit button at the bottom of the page. An OIRE staff member may contact you by phone or e-mail to discuss your request.

Report Definition*Please describe in detail the report that is needed for this request. Also provide the semesters or time frame you need. The more detail that you provide, the quicker your request will be processed. No special characters are allowed.
